ACL/CCL Surgery Cost for Dogs: $2,000–$6,000 (2026)

A torn ACL (called CCL in dogs) is the most common orthopedic injury in dogs over 30 lbs. Surgery runs $2,000–$6,000 depending on the technique, and almost every vet will tell you: this injury does not heal on its own. The question is which surgery, not whether to do it.

What Affects the Cost

  • TPLO is the gold standard for dogs over 30 lbs. It changes the angle of the tibial plateau so the ACL is no longer needed. $3,500–$6,000 including pre-op imaging.
  • Lateral suture (extracapsular repair) works well for dogs under 30 lbs. Simpler surgery, lower cost ($1,200–$2,500), but higher re-injury rate in large breeds.
  • 50% of dogs that tear one ACL will tear the other within 2 years. Budget for the possibility of a second surgery.
  • Recovery takes 12–16 weeks of strict rest. No running, jumping, or stairs for 8 weeks minimum. Physical therapy ($50–$100/session) speeds recovery.
  • Waiting costs more. Untreated ACL tears lead to meniscal damage ($500–$1,500 extra if discovered during surgery) and severe arthritis.

Common Questions

How much does ACL surgery cost for a dog in 2026?
Dog ACL (CCL) surgery costs $2,000–$6,000 in 2026. TPLO, the most common technique for medium and large dogs, runs $3,500–$6,000 per knee. Lateral suture repair costs $1,200–$2,500 and is used for small dogs. TTA (tibial tuberosity advancement) falls in between at $2,500–$4,500. These prices include anesthesia, surgery, and initial follow-up but not pre-op X-rays ($200–$400) or post-op physical therapy.
Can a dog recover from a torn ACL without surgery?
Dogs under 15 lbs sometimes stabilize with strict rest, anti-inflammatories, and a knee brace over 6–12 weeks. For dogs over 30 lbs, conservative management rarely produces good outcomes. The knee remains unstable, causing progressive cartilage damage, meniscal tears, and painful arthritis. Most veterinary orthopedic specialists recommend surgery for any dog over 20 lbs with a confirmed ACL tear.
What is the difference between TPLO and lateral suture surgery?
TPLO (tibial plateau leveling osteotomy) cuts and rotates the top of the tibia so the knee is stable without a ligament. It costs more ($3,500–$6,000) but has the best outcomes for active and large-breed dogs. Lateral suture uses heavy fishing-line-type material outside the joint to mimic the ligament. It costs less ($1,200–$2,500) and works well for small, less active dogs but has a higher failure rate in large breeds.
Does pet insurance cover ACL surgery?
Most comprehensive accident-and-illness pet insurance policies cover ACL surgery if the policy was purchased before any signs of lameness or knee problems. The injury must not be a pre-existing condition. If your dog shows any limping before enrollment or during the waiting period (typically 14–30 days for orthopedic conditions, 6–12 months for some insurers), the claim will be denied. Reimbursement is typically 70–90% after the deductible.
How long is recovery after dog ACL surgery?
Full recovery takes 12–16 weeks. The first 8 weeks require strict crate rest with only leashed bathroom walks. Weeks 8–12 allow gradually longer leash walks. Most dogs return to normal activity by 16 weeks. Physical therapy (underwater treadmill, range of motion exercises) at $50–$100 per session significantly improves outcomes. Expect 6–8 therapy sessions over the recovery period.
